Real Life Options Longcroft/Falkirk Outreach

Longcroft is a registered Care Home for Adults with Learning Disabilities and Physical Disabilities.

This is a small Care Home with a lovely homely environment where people live their day to day life supported with a staff team who are knowledgeable and always support individuals with dignity and respect.

Individuals living in the home are supported to meet their chosen outcomes in life and to live life to the full whilst having their health needs supported by the local District Nursing team and GP.

Falkirk Outreach community support service is a bespoke person centred support service. Staff support individuals in their own home to live independently and support with companionship and wellbeing. Making community connections and providing support to unpaid carers.

The Outreach Service staff are flexible and are adaptable to work with people within their own homes and in the community. They support people to appointments, attend clubs, socialize with friends and family as well as any other domestic tasks they require support with.
